Documentation to support Special Consideration applications

What supporting documents do I need for my Special Consideration application?

If you are awaiting a test result, or have tested positive, for COVID-19 you will be considered for special consideration on receipt of supporting documentation. For more information visit Special consideration due to unexpected circumstances.

All applications for Special Consideration require independently sourced documentation to verify your circumstances.

Documentation which has been fraudulently obtained or altered will be dealt with under the Student Academic Integrity Policy, and may result in termination of enrolment.

Accepted forms of documentation include:

  • A Health Professional Report (HPR) completed by a General Practitioner registered with the Australian Health Practitioner Regulation Agency (AHPRA). Download the correct form depending on which type of special consideration you are applying for.
  • Supporting letter from a treating health practitioner, medical clinic, or social worker on official letterhead
  • Signed police reports or court orders for circumstances related to being a victim of a crime
  • Death certificate or death notice
  • Statutory declaration (for bereavement only)
  • Confirmation of cultural or religious observances, signed by a senior cleric or community leader on official letterhead

Supporting documentation must clearly outline the following information:

  • the date of appointment/consultation
  • the impact of the circumstance upon the student's ability to prepare for, or to complete, the specific assessment task(s)
  • the dates/duration of the impact.

Applications for Special Consideration may be considered ineligible if the supporting documentation:

  • has not been obtained and dated within a reasonable timeframe of the impact period.
  • does not verify that the dates of impact cover the assessment due dates, or that the period of impact significantly hampers a student's ability to prepare for, or undertake, assessment.
  • does not verify that the student undertook a consultation with the treating practitioner.
  • relates to a health condition that the practitioner is not qualified to authenticate.  

Documentation not considered acceptable:

  • Documentation from alternative medical practitioners (e.g. herbalists, naturopaths) covering a period longer than two days, as per the AHPRA guidelines.
  • Letters of reference or support from teaching staff or other University staff including residential college counsellor and Counselling and Psychological Services (CAPS), unless an existing service provision is present.
  • Photographs of the student's circumstances (photographs of supporting documentation as an alternative to scanning are acceptable).

A medical practitioner suggesting a specific outcome does not guarantee that outcome will be granted. Where documentation is not clear or is suspected as being altered, the University will seek to verify with the issuer.
